Create a page to add to my website with googlemaps and zones -- 3

I advertised this project previously but the person who was awarded the project has disappeared and cannot be contacted. A second person I awarded the contract to was also unable to do this. Here is exactly what I want as described in the document uploaded [login to view URL] I am wanting a javascript code for google maps api to embed a map on my site with Bay topsoil location -37.75225813338976, 176.0933796158968 that adds travel zones RETURN TRIPS for 10, 20, 30, 40, 50, 60, and 80 minute return trips, as well as separate zones for Papamoa East, Te Puke and Katikati. Also can each zone have a different colour but still leave the street names easy to read. It also includes the ability to add prices for small and big truck deliveries to each zone according to the table below. Small truck Big Truck Zone 1 within 10 minutes return 50 65 Zone 2 within 20 minutes return 70 90 Zone 3 within 30 minutes return 80 100 Zone 4 within 40 minutes return 90 110 Zone 5 within 50 minutes return 100 125 Zone 6 within 60 minutes return 110 135 Zone 7 within 80 minutes return 120 150 Papamoa East 115 140 Te Puke and Katikati 120 150 I also want a client to be able to type their address in a search box and be given a cost for small truck and big truck. I Have uploaded the table of charges as an excel document as well. The first guy just created a map for me on google maps. Part of the project was for him to add this to a new page on my website where clients could type in their address and be able to see what zone they fell in and how much it would cost them. Price structure is on the page. I need someone to complete this project please. I have installed a trial version of mapmarker pro on the site if this is required. I have attempted to code this myself and an example of this code is on that document above but that gives you the gist of what I want. Please do not bid unless you can complete the job and show that it is functional on a page on a practice website that I will give you access to. I have a google api activated.
