I need a form builder that is created entirely using JQuery and processed using Perl.
I have already found one that is pretty much what I need, but has some bugs that need to be tweaked, and the backend is currently written in PHP. I'd like to have the backend written in Perl instead.

## Deliverables
I consider this to be a fairly simple project, as I've developed form builders in the past. I just don't have the time right now.
The existing JavaScript / JQuery code does have a few bugs that people describe in the blog that need to be worked out. I also need the ability for the "name" of each form element to be added / changed, not just the label.
The backend script must be written as a single Perl script, without using separate required libraries. However, I would like [login to view URL] used to process the data, file uploads, etc...
The resulting Perl script must be able to save forms created with the builder, and also load them back in to be edited.
