Simple mailing script based on php/mysql

I need a simple mailing script that allows me to upload my customer file into a mysql table and for me to send personalized thank you mails to all the customers Script should have the following page to upload txt file into mysql table (see attached for sample) .. page to display all the following 4 entries in the mysql table together with "Sent" status 1. buyer-email 2. buyer-name 3. product-name 4. shipment-date page to run the mailing script Script to pull the above fields from the table and draft an email message for all customers which have yet to receive the email. The email would contain a pre written message that contains some of the fields above. For example Dear Thank you for purchasing from us Thanks
