Survey Monkey Style Survey and Form building HTML5 mobile/tablet device compatible website

This should be tablet/phone/browser (Android/iOS/Windows) compatible with HTML5. I am looking for help with a website that is similar to SurveyMonkey style that allows businesses/users create forms through drag and drop control interface. Forms created need to be stored with a tight ownership and security controls based on the roles and ownerships. Back end can be in PHP/MySQL/CodeIgniter framework. It starts with low level objects.....Questions and Answers.... and Each question have more than one answer possibilities and different types of answers (single, multi choice, image..etc.., like in Survey Monkey). Several Questions makes a Form or Survey and each of this Form or Survey need to be marked in the database and saved as Forms Master List with Questions and Answers assigned list. These forms and surveys are associated with a business and role. One business can have more than one role/account and forms and surveys master templates are assigned at role/account level for each business. Need to store Master list of questions, answers, forms, surveys as individual objects in Database with association to tie them back to dynamically create a form or survey on the fly. Business/role/account Sends Forms to users and users fills these forms. Saved details for each Question and answer will be stored in the database for each user that filled the form. All form or survey and questions and answers and responses will be stored in database tables with relationships. We should be able to regenerate the form or survey that is filled at any time from the database table from saved filled details.... in non-editable fashion. Each assigned Form/Questions and Answers need to have a unique identification for each user/business/[login to view URL] and the filled details as well. Website is built on HTML5.... and therefore the forms and/or surveys can be created either on web interface or from Tablet device or Phone device running Android or iOS or Windows. This Project is on a Critical path and need honest timeline and pricing.
