Participant response survey

Our business provides onsite health screening programs to corporate clients from various industries in Australia. We would like to get feedback from the participants in the program and to this end we used SurveyMonkey to create a fairly simple survey of 10-12 questions with radio button or multiple option responses as well as free text comment options. Because of corporate IT security policy, some of our client companies do not allow access to Survey Monkey through their network so we cant get responses. Therefore, I am looking to have a survey module created that could be linked through our own domain that is allowed access. Ideally, the module would allow for the creation of text based questions and the definition of the response style, the collection of the responses and some level of data analysis for reporting. Access to the survey response module would be via unique URL (using their email address as verification) that would be sent to each participant. In regard to analysis and reporting of the responses we are open to using third party reporting tools if appropriate.
