Simple PHP Survey with Admin Page

I need a survey page hooked up to a mysql database using php, html & mysql. I need the survey radio sections to be required and to give the error message "You must finish the survey!" if they are left blank. I also need the survey with each submit to put a timestamp in the database of when the survey was completed. I need a submit and a clear button which either submits the fields to the database or clears the fields. Upon completion of the survey, I need it to go to the thank you page or back to the form if there were errors. And finally, I need a basic admin page that will list all of the survey fields entered with the option to sort by field and possibly count the percentage of how many picked each question.
