I would like a quotation on a small project. I would like a quiz built as per the screenshare below. The logo, colour fonts, questions and the URL will be different however. [login to view URL] Please provide two quotes: 1 which will include the zip code /utility provider lookup as per the screenshare and another one which will exclude the Utility provider lookup So I know you have read this thoroughly, please outline which technology (e.g. Wordpress + JQuery) you would use to build the quiz and page. Please provide case studies if you have built a quiz before.

Hi I my name is Brian and I am an Australian based Wordpress developer in Melbourne, I can provide exactly what you are asking for using the gravity forms wordpress plugin using conditional fields exactly as you stated. So I can be more accurate, do you have a list of how many postcodes or how many electricity providers you have? Just a rough estimate as i would need to create these values for you. Also what is the software that you are using to have the data captured? I know you have asked for two quotes, but essentially, the two parts you requested would be developed in the same process, the two parts we would be quoting is the conditional fields process and then the styling, which would have the radio button with the providers logo.
