Online Form Builder that feeds into simple CRM

I need an online form creator from which the admin user can setup an application form and send this form to Candidates via a URL web link. The online form needs typical field options like numercial, alpha (short and long), multiple choice, choice buttons, date picker, email, etc. The online form also needs to be able to upload files Once Candidates complete this form and submit, they should get a confirmation email and I should be able to access all submissions as well as uploaded files. The Candidate applications should immediately go into a CRM system and the admin user should be able to access this CRM and add new CRM entries manually. This CRM should hold 2 types of contacts - Candidates and Clients. The 2 different CRM field options are attached in the excel document for this Project. It should be possible to view and upload new files to either of these contact types. The need here is for a recruitment agency who only want a very simple system: online web form sent to candidates, they complete the form, upload files and submit. This goes into CRM system. The admin can view these application and add new candidates details or they can add Client details onto the CRM. That is it for now. Entire system to be hosted on my server
