Build a Website

We have a simple bootstrap site built. Our client would like a 2 page form built that sends data via email and also sends data to a payment processor ([login to view URL]) where the user adds their credit/debit card details. After payment details have been submitted the user then gets sent back to a thank you page or a success/failure page. The form has some dynamic fields and requires some calculations. The form will reside on a linux server, so processing should be done using php and form validation using javascript (jquery). Form Page 1 Design example [login to view URL] 4 text inputs (Name, Business Name, Contact Number, Email) and a continue button Form Page 2 Design example [login to view URL] 1 text input for ‘Number of Employees’ 3 radio buttons: - Auto Enrolment pack - Full Service Package - Contact me first to discuss in more detail Auto Enrolment Pack: Radio input with a value of £385 +VAT (total £462) Full Service Package including 12 months support: Radio input with an initial value of £385 +VAT (total £462) If the user selects this option the form needs to calculate a new price depending on how many employees the user entered. The total price (displayed next to this radio button and at the bottom of the form) along with the price breakdown needs to update ‘on the fly’ when the user selects the ‘Full Service Package’ radio button. Promotional Code: Text input that allows the client to setup both a percentage reduction or a flat rate reduction from the order total. Submit: Sends all collected data to an email address and if the user is making a purchase will send the final value to RSM’s payment portal ([login to view URL]) where the user inputs their payment details.
