I need a modification to the existing multiple check boxes field. This should allow the user to be able to check multiple boxes, and when they need to, give additional information manually with a text box. The text box should be positioned below the multiple check boxes that allows them to enter their text (MAX 300 words). Please see attched image for how this should look.
