Website customer and production registration form

This project is for a website wanting an informal product registration form. The form will collect customer and information on the purchased product including an option to sign up for a newsletter. Prior to be submitted, the registration form will have validation for required fields and a challenge response entry to verify entry is generated by a person (i.e. captcha). Information collected will be entered into a database. The collected information will be used for newsletter mailing and market research. The webserver where the registration form will be hosted is CentOS 4.7 that supports PHP, Perl, Python and MySQL database. The website [[login to view URL]][1] is also running SMTP and POP.
