Make an oder website(repost)(repost)

I already have my website but now I need an area where people can put an order online on how they want their salad to be prepared and send it to us via E-mail. We have 3 different sizes of salads (Small, Medium and Large) Customers can design their own salads. Salads can be sold separately or in Combos. Combos include Salad + Flavor Water + cookies. ingredients for Salads are divided in 4 specific groups: Premium Ingredients (12 different options) Basic Ingredients (30 different options) Topping Ingredients (14 different options) Dressings (11 different options) Ingredients included per Salad: Small Salad (1 Premium +3 Basic +2 Toppings +1 Dressing) Medium Salad (1 Premium +4 Basic +2 Toppings + 1 Dressing) Large Salad (2 Premium +5 Basic +2 Toppings +1 Dressing) If customers choose a Combo they are also able to select between 2 different types of water: Flavor Water (2 different options) Water can be changed for a Soda paying an extra charge. Cookies (6 different options) Customers are also able to add extra ingredients with an extra charge depending on the type of ingredient (Basic, Premium, Topping or Dressing) They are also able to add extra cookies with an additional charge. Also they are able to add more things to their order like coffee, bagels, sodas, etc.. included in the Menu. Customer can be able to select the store from where the salad will be picked up. (Actually 2 stores) Customer has to give their name, phone, address and zip code to be able to place an order. Customer can choose between Pick Up Delivery or Home Delivery. Program must be done to work in a Linux Server. preferably on PHP.
