Grapheme/Text to Phoneme Converter

I need a form with an input field which runs a script which converts the word or words entered by the visitor into a phoneme or set of phonemes as defined by the dictionary available here: [[login to view URL]<wbr />cgi-bin/cmudict][1]. I'd like to add this to my Wordpress website ([[login to view URL]][2]). I would like the design and look of the converter to match that of my website (Linen Theme + Blue/White/Grey color scheme).
