HTML Travel Booking Form and Web page map site

Hi, I have a project that I needed to finish as soon as possible at least within 24 hours. I really need this ASAP. My project is a Booking Form used by travel agency, when users send the form in PHP format.. should write into a file using a PHP code then lists all the total number of travelers, their names, flight destination, etc. (I have the layout of the form). Additionally, I need calendar included in the form...just like it was in the booking form, where users can select the date, (should only have one way destination). Also, I need an image map of my webpages where their is a hot zone so users can get the information about my web page by just clicking the hot zone. I have the sample codes that you can use to generate my requirements, but the rest will be your codes. I will provide form layout and some codes that you can use..its up to you use it or not or use your codes but would rather that codes should be at least similar to the codes I will provide. And codes must be understandable by beginners in programming like me. If I can't understand the code you use, you have to change it. I need it to be working codes. You must validate the form, javascript code, html strict DTD, and CSS. You must have local server to test the php code, at least a photoshop or any image editor to create a hot zone point. I have the sample code for this for you to use. Ask for zip file...there may be more instruction to discuss, so we need to communicate through skype.
