MySQL Database Document Control System

**MySql Database System for Document Control Management**. Approx. 1500 - 2000 Documents Formats: Word, Excel, PDF, Visio, Power Point Admin Screen (Add/Edit/Void) View/Print Reports Currently managed via spreadsheet and hyperlinks. Scope attached, feel free to request additional information. Note1: Current system has each facility managing the same forms separately on facility hard drives. We are standardizing the forms and centralzing the access point via internet. Note2: The new document control log does we were using to consolidate forms does not allow for 1:1 relationship between retention/maintenance information for Forms control (a form may be applicable to more than 1 facility, as such, each facility must define who maintains it, for how long, where, etc.) This needs to be addressed in the table definition. Note3: The new document control log does not allow for a cross reference to the current facility level reference ID. We MUST add this to the table definitions. EG: New ref id = BBi-40000, Ajax ID= FRM-XX-XXX, Holland ID = QP-16-001, etc. Note 4: Company website is at [[login to view URL]][1] application must use graphics/colors of company. 053104 Update: This is a system to manage documents (procedures, forms, exhibits, etc.). Users within the organization will be able to access the most current document (print, fill in, etc.). Documents are to be uploaded to the system along with a header record containing document description information. System exists primarily to satisfy ISO document control needs. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Platform: Linux/Apache Server Code: Php interface to MySQL Database Users: Can be up to 100 users in system at a time (more likely 6-20)
