Testing Process Application

I need a form created of questions for vendors to fill out. When a vendor comes to us with a new product they would first have to fill out this form for us to elvaluate their product. The form would consist of the ability to upload documents into specific areas of the form and have questions and answers on other areas of the form. Then the form would route to the approval department. Where they would evaluate the documents and vendor and update the form based on information and findings they have. They would decide to evaluate the product or no evaluation at this time. Then we would have a form to fill out on what the next steps with this vendor would be. The vendor would be able to be emailed from the form at any point with a status of where they are at in the process. I need to be able to intergrate this into our corporate website.
