436855 Create multi part form and thankyou page on website

I need someone to create a multipart form on my website [login to view URL] . The current form does not work. It should collect all details from 3 parts and email it to a number of people. I also require a thankyou page in similar style of the site or a thankyou message to appear in the same place as the box once details have been submitted. Really easy job for any good PHP developer.
