Complete a template based website - editable tables and form validation core- HTML5/PHP/MySQL/Javascript -

I have an incomplete website/application design (80%) based on this template: [login to view URL] , and a MySQL database already designed and populated with some sample data, the data model is simple, essentially one central table interacting with some catalogs using ID's, + a users table. The main goal of this application is provide different users, the capability to use between 4 form wizards and some "business rules" (simple mathematical calculations) to generate a pdf, store it in the file system and insert a row in the database. A couple of interfaces to select data using form-filters like dates and names, and use html tables to retrieve and edit rows are also required. You'll need to use javascript form validations, php-MySQL and html5 as necessary in order to make capable insert/update/select the database using forms and table interactions. Some minor interaction with widgets like counters and simple charts using query results is also required. (the design of those specific elements is already done). As said, the general design is around the 80%, so you'll have to complete the reminder based on the existing pages (pretty much the same style).
