Photoshop Color Photo into Black and White with Black Background - PRETTY SIMPLE STUFF -- 2

I need the enclosed iStock_000077748 photo to be photoshopped to where the tall brunette model in the photo is isolated, turned into black and white, and placed into a black background. I need a professionally done job with all her hairs still intact and perfect down to the final detail. I am trying to create a Victoria Secret like poster for a client which you will see enclosed as IMG_1015. The picture has to fit nicely into a 42\" x 21\" format, so please adjust the image size so it fits nicely in that size, we want all of her: hands, head, body, and full legs in the picture.... please dont crop any of that.
You\'ll see in the picture Models 6 for Doc Photo how that pic is black and white with the black background. We want the picture you\'re photoshopping to look like this effect. But again the one you\'re doing will have her full body, head, and everything.
This all shouldn\'t take too long for anyone who knows how to use background eraser, I just dont have time to do it here at a meeting.
