Simple Ipad app to replace paper forms with online forms.

Create a simple app so our teams can save paper and enter information from their ipads directly. Our quality control (QC) team visit factories and have to fill in standard paper forms. We want to upgrade our system so they can automatically fill in forms using an ipad, and that the head office can then generate a word document based on the answers and pictures. Basic functions: - Questionnaire (check-boxes, drop-downs or text entry fields) - Locations (we want to be able to track users location during factory visit) - Expense report (simple function where qc team can add expenses, very basic) - Pictures (ability to take pictures and add them to the generated form) That's it.
