Logo conversion (web & print format)

I have a logo designed already that needs to be converted into files that work well online and in print. The logo is 99% text. I need the logo created using the latest vector tools that can be resized without losing any quality for use in the following: t-shirts, web sites, billboards, business cards, letterhead, cars, trailers, bus, etc. I need a very small (biz card), a small (letter head), a medium, a large (t-shirt), and a super-sized file of the logo that can be easily scaled down in a graphic editor to a anysize. I need it in the following formats: AI, EPS, JPG, PDF,Gif, Tif, PNG, PSD I also need a gray scale version, a transparent file and an editable source file. Additionally, I will need a document stating the release of full rights and copy rights, etc.
