Design a dynamic form plugin in jQuery / Javascript

Looking for a developer to design a dynamic form javascript / jquery plugin that can populate a form from a JSON source using AJAX. Form must be able to support the following control types -> Standard Text Input -> Radio Buttons -> Drop-downs Standard Text Input must have the ability to mask and auto-format for: -> Date Input - using bootstrap date-time picker. -> Currency - formatted with a 1000 separator and masked to 2 decimal places. Radio button / drop-down selection must be able to trigger and add sub questions to the form. Input may or may not be required - will be defined through JSON feed. Form must be user-friendly using bootstrap css layout. Form to be submitted through AJAX. Ajax Source may be able to respond with validation errors to be displayed next to relevant form input. Ajax source must response with success or error message upon form submission. Success or errors to call functions. Developer to provide the JSON formats for all of the above AJAX responses.
