WordPress CUSTOM design . . . only 5 pages total

Hi I am looking for a Professional Designer to build a custom wordpress website in 14 day turnaround OR LESS. Work Includes up to 5 pages including adding Images in pages , header display with jquery slider on home page and static hero banners on other pages, Form Submission, keywords, google analytics, loading site on new servers. I like the twitter bootstrap and responsive type themes, so functionality and feel should be close to those. I will require this to have a clean mobile version. I do not mind plug-ins for the mobile version as long as it works to my specs. I will require support guaratnee for 30 days after completion in addition to being WELL documented code so that I can edit content in the future. I will provide color theme and logos. Please send 5-6 of your best websites in WORDPRESS. Bids above 75$ WILL BE IGNORED. I work for an ecommerce company and know that this is a few hours of work at best. I will provide you with a PSD mock up so you will know EXACTLY what I want. This is an easy job for someone with wordpress experience. I am not looking for ready made theme, should be CUSTOM MADE.
