Create template for use on an iPad

I would like to create a 3-4 page template for a cosmetic surgeon to input exam information on patients and then include pre-operative photos of the patient. The entire exam and photos should then be printable for the chart and the patient to take home. The template will include boxes that can be clicked on to check and blank fields that can be typed on for additional information. I want to confirm that you are comfortable creating this template for an iPad that allows us to check boxes, input notes, take photos on the iPad and attach the photos to the chart note and then print the file and email the file and allow the file to be sent as microsoft word file, pages file, or pdf.
