I need to create a simple on-line "assessment" tool. This consists of a Web page with about 20-25 questions in a few different categories. Based on the response to these questions, a results page will be shown with a "score" for each section and comments. As this assessment will also be used on an iPad in future, it would be preferable for a Web API to be created that takes the scores and outputs the results to a web page. I've attached a sample of the questions in Excel. I will provide: * Final check-list in Excel * Logic for the questions and results * Mock-up of what the output (results) page will look like
