We started a project 6 months ago to computerize the DMAS-90 form. You can view what has been done so far by watching the video's at www.dmas-90.com. We took the product to market and found out that this form needs to be filled out by an IVR system. We have setup Asterisk for the IVR system. It is setup on Debian (lenny). We have the in coming call routed to the asterisk server from and IAX connection GSM. The DMAS-90 is running on a Windows box on IIS6 with PHP and MySQL. We need someone that has strong skills in this area to pull these two systems together and finish the IVR. We have clients waiting for this project to get finished. If it can be done in 5 - 10 days that would be great. I have attached some documents to show how the system is flow. Also, there is a basic list of the IVR questions to get the system up and running. If you have strong skills in IVR, Asterisk and PHP, we need you now to finish this project.
