Converting word templates to html forms

We have several word documents that needs to be converted into html forms. These forms need to run on a IIS v5 webserver. We like a design with CSS and limited use of scripting. We would be using a form method="post" to post the file to an ashx file We have attached an example of the kind of templates we would like created and will submit the remainder to the successful freelancer.
