Software program that creates advanced pop ups needed

I need a software program created that will create pop ups. Data entered in fields on the software program would be automatically inserted into some html coding. This will also have sound incorporated into it and must create needed .js files I have several different software that you could model after that will be combined into one but this one will have some differences and more features. Please let me know any other information you may need to know and I will PM back. I have already had 1 time waster so only people who have created programs and can SHOW ME THESE PROGRAMS will be considered. Please note this is not a simple pop up and requires sound and .js files. I am open to the idea of this being web based instead of desktop based.
