I would like a form designed with plugin Gravity form. Approximately the form will be letter size amount of text. Later I would like that when the person who has entered all the info hit's send, he of she gets the copy of the information in a nicely created PDF by email and we also get this same copy by email. The PDF should have company logo in full color, and all the filled data in a nicely and presentable format. I have several of these safety forms to create but at the moment I just want to try one of the forms out. Secondly I do not want any person to come to my website and start filling these forms out. I would want my clients who have log in access to be able to do it only.
