Our question here is how would they get an account on the site ? do they need to register themselves to have an account? or will you register your patients yourself and then share the username and password with them so that they can participate in the survey? The survey results would be stored in a tabulated manner in the database which can be used in the future as per your requirement and can be extracted from the backend (Admin panel). What other features do you want on the site? What are your thoughts about the look and feel of the site? How many questions do you assume to be there in the survey?
