Make an automated flow on Jotforms/Google Forms

I currently have a internal violation tracker: [login to view URL] I want to make this form into an automated email that is sent to the violator and puts them on notice and requires them to sign and acknowledge the notice of violation. I'm thinking of making a flow like this: 1. I fill out form (Jotform or Google form). I have a paid JotForm membership 2. Form turns into an e-mail w/ my electronic signature 3. Email is sent to the violator for a review and signature 4. Violator signs the form 5. It comes back to us and the violation is tracked on a spreadsheet
