HTML5 Contact Forms - Need 2 forms connected to our hosting platform's database

Hello, I have a few html sites that I need finish up some contact us forms for a small HTML5 website with 2 forms: 1. a LEAD generating form with basic user input fields: name, email, phone, message, policy number, and a selection of "new" or "old" claim. Also, we need to pass the type of claim based on the page the user is on the page the user is on when they complete the form. 2. A generic contact form. These forms must generate a notification email and must write to the provided database which the client can retrieve easily as needed.
