Encode a simple picture upload and validation interface in PHP (Read attached project document)

I have a Web developer currently working for me on a website design project using PHP language. To complement what he is doing, I need a freelancer well versed in PHP to design and encode in PHP language, a dynamic picture upload and validation interface that I intend to use on my website. Please note I am not able to discuss any details of the website with anyone. All the details required for this project is contained in the attached job document. Please read and familiarize your self with this document before bidding. The development and testing of this interface will be done on the freelancer's website. All I require in the end is the PHP source files to use on my website. I only look forward to working with freelancers with a good command of English.
