Create Simple PHP Online Reporting Script Based On Excel Doc

This is an easy job for a good coder who knows what they are doing. I need to recreate the report attached online but allow each person (in the attached spreadsheet to have their own page and then be able to input their info into it and then it will calculate the numbers and % on a master report (same as the report attached) Each person who input their data should have their own page where it lists all their clients and has then each data input field for the columns on the spreadsheet to input the relevant , info, yes, no , number , comments etc for each client. Once all this information is input it then it will total, add up , works out percentages for all the total persons input on the master report. Once all this information is input it then on a 'master report' totals, adds up , works out percentages for all the total persons input on the master report. We need this developed on your server then transferred over once its complete. I envisage it will be a PHP code set up and with a mysql database. The area must be password protected and the design would be simple white screen with our logo attached at the top right hand side of the screen. I would like to use the 'Helvectica Nueve' font throughout. If there is anything that is not clear or you have questions please ask. If you are submitting a bid and have done similar reporting work then this would be looked on favourably with your bid.
