I need a web page to register club membership and take membership payment via Paypal. After initial online registration, the users email account must be verified prior to taking payment. The membership total will be calculated according to which boxes have been checked on the form which will be supplied to you prior to this project commencing. After successful payment, the member should be emailed a PDF 'club card' so they can print out and present to club sessions. The administrator should be emailed a PDF form containing the user details for hard copy print. This will require some layout skill in FPDF ([login to view URL]) or similar. A sample expected layout will be provided and will only be a single page. All user information will be stored in a mySQL DB as future enhancements utilising these details will be required. Do a good job with this and you will get all future work from us. Please note: NO FUNDS WILL BE RELEASED FROM ESCROW UNTIL MY CLIENT IS SATISFIED THAT THE BASIC OVERVIEW ABOVE HAS BEEN SATISFIED. FURTHERMORE, IF YOUR RESPONSE TO THIS JOB IS GENERIC, IT WILL BE DISCARDED.
