Build Page to enter data and provide an up to date list of all data on the success page

I need to create two separate web forms to accept data. Upon entry of data, the success confirmation page will also show all prior data in the database. 1. Citations I need a web form for users to enter data (citation number, date, time, violation, location, license plate, make, model, and color). Fields should be checked for accuracy or converted to a standard format (date, times, license plates, etc). Some will be pick lists (location, color, make). Upon saving the entered data, the user should be returned to the page that includes the last 1 year of records (today's date minus 1 year). This "home page" should also have a search field where users can search by "license plate". This license plate search shall find two categories: exact matches and close matches (+/- 1-3 characters in case of data entry error). The user should not have access to modify or delete data. We need an administrator login where all records can be viewed, updated, deleted, and maybe even a CSV download for a certain date. I have approximately 850 existing records that need to be imported and can be provided in CSV, XLS, or any other formats. 2. Pass Downs I need a web form created for users to enter data (date, time, location, incident). When the data is submitted, an email is generated to notify. Upon entry of an incident, a case number is automatically generated by the system and added to the beginning of the record. After entry, on the success page, the user is also redirected to a page that shows prior incidents that were entered in the database, along with the same page for users to enter the next incident. Finally, the database should have an administrator access. This way all records can be viewed, updated, deleted, and maybe even a CSV download for a certain date.
