PHP - Event Registration Script

Hi There, I need a freelance that has good PHP and HTML skills. Im looking for a developer to create an event registration page for me. They can use existing scripts like the ones here: [login to view URL] I will purchase this script for the developer if they need it. so they can use it for the new system and add my additional requirements The designer has to create a custom webpage(s) which looks and feels the same as our main Site [login to view URL] ,. below are the pages required: Logon Page Fields: • Name: • Password: Registration page Fields: • First Name: • Surname: • Email: • Password: • Confirm Password: • Registration Key: Once a user has registered they are logged in straight away and brought to the welcome page. Welcome Page: The welcome page must look like the drawing attached and also say: “Welcome to the Event and have their “Name”: It should list the available events to register for Event Register: When a user clicks to register for an event the following details must be captured. I want it 4 sections: Section 1: Event 1) Attending: Yes / No (Tick box) 2) Location: Galway / Dublin (Drop down Menu) Section 2: Transport 1) SAP office to daytime Activities (Tick Box) 2) Galway to Dublin Return (Tick Box) 3) Guinness Store House to SAP office (Tick Box) Section 3: Accommodation Text: Accommodation for the Galway staff is free providing they share a room. There is an additional charge for signal occupancy. Dublin stage will have a special rate at the hotel. Require accommodation Yes / No (Tick Box) Sharing Room Yes / No (Tick Box) Name of room sharing colleague: ____________________ (Text Box) Section 4: Specific Diets Vegetarian: (Tick Box) Vegan: (Tick Box) Celiac: (Tick Box) Gluten Free: (Tick Box) When a user has entered in all there details there must be a submit button and they will be brought to a review page. Event Review: This page will be a summary of all the entered details. Once the client is happy they will click on the “Register” Button. Email Once a client clicks on the register button a email will be sent to them thanking them for registering and also a summary of their details. Admin: [login to view URL] Username: admin Password: admin In the back end I need to be able to edit individual attendees details. So if they want to make a change the Admin can do so for them. I have included two images of how the "Welcome" and "Registration Page" should look. If you need better drawings i can supply them. As these are quite rough. Looking forward to working with you.
