533827 Music Exit Poll

Hello to everyone, i need one way to have this project... i have 50 songs and i need to make exit poll for that songs to make a winner... i need one form with option to add 50 songs titles with audio link (mp3 player) in this form i need my users can hear the songs and vote 5 songs per day... also i need a way to see how much users vote and how much votes have every song... if you have any suggestions please send me an email... i need it immediately.. thank you
