Magento 1.9 Contact form set up + upload the theme in 5 magento almost mirroring site

Magento Contact form have mixed fields for retailers and wholesalers clients. There are 2 fields ( tax id and occupation) which must be presented only for wholesale contact client and not for retail contact client. When the client is wholesaler then he choose wholesaler and only that time must be presented the 2 additional fields ( tax id and occupation). If the client is retailer then the contact form must be as it is now without 2 additional fields for the wholesalers. Also the main Contact form in Magento 1.9 responsive theme must be add to the responsive menu and the "send the form" button must be shift on the left to become visible although from tablets, ipads, smartphones. In the main magento eshop there are 6 similar Contact forms which linked with the mirroring sites must be set up also. Attention the contact forms are linked also in database web application so every field is referred in specific table in online database web application so this code must be remain as it is because otherwise the contact forms will not be saved in this database application. Then the existed theme must be upload in 5 magento similar ( almost mirror) content web sites
