The following three forms require improved layout and performance: 1) [login to view URL] 2) [login to view URL] 3) [login to view URL] Forms 1&2 are identical in function, only the names are different. Explanation of Utility: The site visitor will access these forms for the main purpose of uploading files along with the relevant information. Chicago City Graphics (CCG) will receive an email announcing the form's submission, which includes a link to download the submitted files. Problems to be addressed/corrected: 1) There are a lot of necessary fields in these forms, making them quite long and unattractive. I would like to see the forms segmented into parts, using either tabs or multiple pages to collect parts of the information separately. For example, users could enter personal info (name, company, email, etc.) on one page, then move along to submit the next data. The more modern the technique, the better - looking to minimize page reloads and have 'cool' presentation. 2) The files that are being submitted can be VERY LARGE. Currently, the forms are having trouble with files over 1MB. This is not going to work, so we need to identify what is limiting file uploads, determine the max file size that is possible, and allow large files through. We are using PHP and MySQL to build the site, although the current forms are not utilizing any database functionality. I need someone who can decide whether what is already there is worth adjusting, and then get these forms working as requested.
