Simple web forms for user study

We are looking for a skillful developer to help us build a series of web forms to collect user study data. We want the forms to be based on Java technology. In each form, we want to show 15 images. A subject will select 3 images. The data will be stored in SQL tables. The project must be managed by Maven and can be deployed by a simple web server such as Jetty. Templates based on the StringTemplate library is preferred. Please see the attached document for more detailed description of the forms.
