We would like to add a map feature so clients can select their site location and we can calculate mileage (and know site location) based on that. Would be best if there was some ability for us to customize the survey (e.g. adjust wording, adjust pricing) independently. We need to be able to track any responses so we can follow up with those clients. I was told that the programmer should be able to use Jquery and the Google Map API to create what you want. This may also involve liaising with the company who currently manages our website to make sure it is compatible with their needs. We are in early stages but would like to move this along quickly. I have attached rough ideas of what might be a portion of it. The attached is for a Phase I ESA - we would have a similar (but likely more complex) version for a Phase II ESA as well.

Good Evening, what you are after can achieve through Google map, but that will be guess work arial distance between two points, I am going to suggest you to use Google Driving Api to get correct driving distance.

Hi there, I can build this for you, already done a very similar thing which included calculating the distance on Google Maps, I can show you a demo of that. The budget is higher than I would bid for something like this, so I'm very eager to work on it. I haven't seen Phase 2 yet, but I'm pretty sure this budget would cover that too. As you were told I would use Google Maps API and jQuery (which is a JavaScript library). If you'd like I can write it without jQuery as well, using plain JavaScript.
