No cover page necessary, just the form. 3. form adapted from questions in columns of [login to view URL] with heading "Employee Hours Reporting Form". No cover page necessary, just the form. 4. an injury poster like this [login to view URL] adapted with information from [login to view URL] (please don't copy artwork - just used for an example of the type of work i'm talking about) 5. redo of attached [login to view URL] to match your style Requirements: 1. fluent english speaker as they will need to adapt text from the website & other forms without me telling them the exact text 2. final product sent in indesign with any source files & fonts I'll will provide logo in .ai over pm
