Create Userform for patient data entry

Greetings! This is a fairly straightforward requirement. Being a full time freelancer working exclusively on Excel based projects, I work on such Excel based tasks every single day. Please share the existing files - I would update VBA code so that using unique identifier, new data can be added to appropriate columns. Important: Since it is confidential data, I would only need a blank file with macro code and the list of columns that need to be update. For testing purposes, I can rig up data at my end. I have 20 years experience in using Excel as part of my daily work life. Over the last 30 MONTHS , I have spent at least 200 HOURS on automation in Excel every month. Have a Great Day!
