Convert PDF School admission application into web fallible application

I own a preschool/daycare and every time I need to enroll new child into our school, the parent of the child have to fill 40 page enrollment application which takes them several hours to fill out the entire application. I have attached the application in pdf form. I want new students to fill out the form on computer, tablet or smartphone. Here is what i am looking for 1)Convert pdf file into an web application that parent can fill out faster 2)These documents have numerous duplicate fields, I want the parents to fill just one field and all duplicate fields should be auto filled. For example, the child name is on 5 different documents and the parents have to refill 5 times. 3)Once the parent fills out the application, I want the data pasted in the appropriate field in the pdf document. 4)I want to have the ability to edit applications if it is incorrectly filled. 5)Save unfinished applications and restart the application where the user left off without losing the data. 6)Have the parents electronically sign the application like the software called dochub or docusign. 7)If the parents have more than one child, give the user the ability to select the number of kids they like enroll and ask them for their information. 8)I have attached 3 pdf files and all those need to be combined into one file. 9)If the child age is 1 day to 2 year, the user needs to fill "Genius Kids Infant Package 2020" and "Meal Benefits form for children.” 10) Depending on the age of their children, for example, infants (age 1 day to 2 year) require additional documents to be filled out. So if a child age is entered under a certain age, infant specific documents should be required to be filled out. 11) Parents to needs to fill out all required fields.
