Design work

HI everyone, We currently have a component which we're trying to get onto code canyon however we don't have the best design and our usually designer isn't great with forms. The component is: [login to view URL] It allows a developers / website owners to easily style their dropdowns. We've had a few sales but obviously design is an issue. We're looking for someone who can design 3 website forms with dropdowns inside them. - Contact form - Product Selection form - Search form The content isn't necessarily relevant, it's more about the design we want 3 designs which look modern, clean and attractive. Mockups would be appreciated. Please don't send us hundreds of examples you've done before it's irrelvant what is relevant is this project. End product MUST contain complete layed PSDS and the rights to use them as we see fit. Thanks
