A Few Forms For Data Entry in PSD / PNG

i have already ready form style. I will give you the ready form in PNG. I need this forms for my handshakes community site. I have drawn the pictures and scanned it. What i want is very easy but its hard to explain but it wont take more than one hour. You just need to type it and create it.
