Small and Simple 3 Page Web Management front end for 3 MYSQL Tables

I need a simple no-frills management web front end made for a small 3 table database. (total of 20 fields in all) This admin web pages will manage a game license application I have made. I have resellers who need to use this function to login and manage their account, and perform a few minor functions. Software Requirements: (you can use any versions of below, as this is a very simple interface) Apache/2.2.14 (Ubuntu) MySQL client version: 5.1.41 PHP extension: mysqli Total 3 web pages need to be created 1) Login Page Admin Login with email and password. You can create a new table to hold Admins email address and password (MD5 hash). I will manually enter them into the database as there will only be < 5 total that would ever be used. 2) Main Display Page: After login it will show a table with Columns (CODE,GAME,STATUS,ACTIVE_DATE) with records pertaining to resellers email address only. 4 Functions per row can be performed, Hardware Reset, Disable,Enable, History (see #3 below) Hardware Reset: Takes "ID", and sets "STATUS" to 3 Disable: Takes "ID", and sets "STATUS" to 4 Enable: Takes "ID", and sets "STATUS" to 1 (only if STATUS was 4) History: Displays History page with all hardware reset information (see #3 below) 3) History Page There is a history table that keeps track of all hardware changes, I want to display all these columns found by "CODE" when they select the activation code from the Main Display page (above) Upon acceptance I will give all sql queries and sql executes to be performed for each of the above. You wont have to figure out any of the logic.
