Design a PHP Login Page with SQL

I need minimalistic HTML, I only need the PHP/SQL setup. There WILL be future work for this. Register • First Name Textbox • Last Name Textbox • Email Address Textbox • Employee Number Textbox • Team Leader Dropdown • Job Role Dropdown 1 The column "account" which by default records all new users with the value "User" 2 The column "status" which by default is Active. 3 The Team Leader Dropdown is to read from the logins table where the column account has a value of "account" 4 The Job Role dropdown will have (Legal Assistant, Solicitor, BS and Other) Login • Username Textbox • Password Textbox 1. Link to reset their password. 2. Update last login to reflect when they last logged in. 3. If they are disabled do not let them login (show an error) Forgotten Password • Username Textbox • Employee Number Textbox 1. Employee Number must match up so it can be reset. 2. An email is to be generated with a random password (mixture of capital letters and numbers) 3. When their password is reset please update the Last Password Reset column to reflect this. 4. If they are disabled do not let them reset their password (show an error) Administrator Page 1. They will have the function to change their password. (Must contain at least one capital and one number with a minimum length of 8 characters). 2. List all users line by line showing their first name, last name, email address, team leader, *account*, employee number, job role, last login and last password reset by alphabetical order by first name. Show Active and Disabled (please show disabled in red and active in green) 3. For account create a dropdown which will allow you to change them to user, team leader, reviewer or admin. An example of 2 users in the database in which the admin can change the account type from the dropdown quickly and click submit for ALL users. If the tick box is ticked the please change the status column to disabled. User Page 1. They will have the function to change their password. (Must contain at least one capital and one number with a minimum length of 8 characters). 2. When they login they see who their teamleader is and can see a dropdown of all teamleaders from the logins table > Account column in which it reads all with the value teamleaders. Please filter any names that contain under account the value "reviewer" to be excluded and any disabled users. Team Leader Page 1. They will have the function to change their password. (Must contain at least one capital and one number with a minimum length of 8 characters). 2. When they login they see who their Team Leader is and can see a dropdown of all 'reviewers from the logins table > Account column in which they can change their teamleader. Please exclude any disabled users. 3. List all users line by line showing their first name, last name, email address, and job role. Please filter so it only shows their users who have them selected as team leader and filter alphabetical by first name and hide any users that are marked as disabled. Reviewer Page 1. They will have the function to change their password. (Must contain at least one capital and one number with a minimum length of 8 characters). 2. List all users line by line showing their first name, last name, email address, account, employee number, and job role. Please show teamleaders and then with a new list show all users with their teamleaders.
