Custom Website Programming

I need a website that does the following: 1) is MySQL/PHP based. 2) automatically create webpages from the information inside the mysql database. 3) I want to upload an excel or csv document into password protected admin panel. The cells will be: company name, address, city, state, zip, phone, website address. 4) The script will create a new webpage for each company as follows. [login to view URL] 5) I will give you an html layout that each page is to look like. (only variables that are changing are the variables from #3.) 6) On each page there is a contact form that is to save into another mysql database that can be accessed from the admin area. I also want the form information to be emailed to me. We will be collecting name, email, phone number, and state. 7) Based on the state that the person entered, I want a different automatic email response going back to the submitter. 8) There should be a site map that shows all the pages in the site.
